Role of the RT in Patient Assessment

The role of RTs in patient assessment has changed dramatically over the past 30 years. RTs now assist the physician in determining the initial diagnosis and play a major role in assessing the patientâ€s response to therapy. The stages of patient-clinician interaction is listed below.

Stages of Patient-Clinician Interaction

Pre-interaction Stage1. Review the patientâ€s chart to identify important facts stated in the medical history.

Introductory Stage1. Introduce yourself to the patient stating your name, department, and purpose of the visit.2. Confirm patient identity using his/her wrist ID bracelet.

Initial Assessment Stage1. This stage is done to identify the current health status of the patient. A brief interview and physical examination are appropriate.2. Donâ€t assume the patientâ€s condition has changed since the initial workup by the physician at admission.

Treatment and Monitoring Stage1. Provide treatment and monitor patientâ€s response.

Follow-up Stage1. Take a minute to ask the patient about how he/she feels and if you can do anything else before you leave.2. Also let the patient know when you will return.

Communicating the Assessment Findings1. Record details of the patient encounter in the chart.2. Use of Space

Social Space (4-12 ft.)1. The social space is useful for the initial introduction of yourself to the patient. Social Space should be kept between 4-12 ft. between you and the patient. 2. This is the first step toward establishing a rapport with the patient.

Personal Space (18 in.-4 ft.)1. The personal space is most useful for the interview. Intimate Space (0-18 in.)2. Typically only invaded after a rapport with the patient has been established3. Useful for the physical examination4. No eye contact in this space

Territoriality1. Patients often “lay claim†to items in their room that are placed bedside even though the items belong to the hospital. For this reason never take a bedside item from Patient A and give it to Patient B without obtaining Patient Aâ€s permission.2. Respecting the territorial rights of patients is an excellent way to let patients know you value them and that you are considerate of their needs.

Confidentiality1. All patient records are confidential. What you read in the chart and what you discuss with the patient must be shared only with other healthcare workers who “need to know†the information to perform their job appropriately.2. Discussing patients in areas outside private reports rooms must not be done.

Expressing Genuine Concern1. Face the patient squarely2. Use eye contact appropriately3. Maintain an open posture4. Consider appropriate use of touch5. Be an active listener
