Please write all 3 pages and I am working with a family of 2 a 76 year old lady that has hypertension, diabetes, hypothyroidism, rheumatoid arthritis, tachycardia, among other diseases and her daughter they are Hispanics. and for the community health place I am attending Your Day Adult Day health care center where most of the participants are Korean and Filipino and some of them have either hypertension or both. let me know if this information is helpful in order to complete the assignment or in case you need more information let me know, i reside in San Fernando Valley and this is also the place where the family and community help center are located as well.
